新辅助化疗对Ⅲ期非小细胞肺癌纵隔转移淋巴结的降期作用

摘要:
目的探讨新辅助化疗对Ⅲ期非小细胞肺癌(non-small cell lungcancer,NSCLC)的纵隔转移淋巴结(mediastinal lymph nodes metastasis,N2)的降期作用。方法回顾性分析接受新辅助化疗的43例患者的临床资料。结果N2降期患者的预后较好,而N2是否融合对于降期与否有显著影响,患者的性别、年龄、肿瘤的病理类型、化疗一手术间隔时间和化疗方案与N2降期与否均无关。结论新辅助化疗能够显著改善Ⅲ期NSCLC的手术治疗效果,而单个、孤立的N2患者有可能获得更好的降期疗效,进而获得较好的预后。

Abstract:
Objective To study the down-staging effect of neoadjuvant chemotherapy on mediastinal (N2) lymph nodes in patients with stage Ⅲ non-small cell lung cancer (NSCLC). Methods Clinical data of 43 stage Ⅲ NSCLC patients with neoadjuvant chemotherapy were reviewed retrospectively. Results The patients with N2 down-staging had better prognosis than others. Mediastinal lymph nodes fusion was a significant factor on down-staging outcomes. Meanwhile, gender, age, tumor histology, interval period between chemotherapy and surgery, and chemotherapeutic regimens had no obvious influence on N2 down-staging. Conclusion Patients presenting with single or solitude mediastinal lymph nodes metastasis have better down-staging effect and better prognosis by means of preoperative chemotherapy.
