Objective To
study the clinical features, diagnosis and prognosis of simple primary medullary
hemorrhage.A case of simple primary medullary hemorrhage hospitalized in our department
was reported, and all literatures, domestic and abroad, published since 1964 were
reviewed.Results Simple primary medullary hemorrhage has an abrupt
onset.It is characterized by dizziness (including vertigo) and headache, accompanied with
symptoms and signs of impairment of long tract and posterior cranial nerve
group.Cerebellar ataxia and nystamus are commonly seen.Head MRI may reveal abnormal T1-
and T2- weighted high signals within the hemorrhagic region of the medulla
oblongata.Conclusion Arteriovenous malformation, cavernous angioma and
hypertension are three main causes of the disease.Cerebral angiography examination is
usually normal.The definite diagnosis mainly depends upon head MRI.The prognosis is not so
critical as traditionally expected and its fatality rate is relatively low. |
